Position Summary
Heart South is seeking a dedicated Advanced Practice Provider (APP)—Nurse Practitioner (NP) or Physician Assistant (PA)—to join our outpatient care team. This role provides comprehensive patient care in a non-surgical outpatient setting, including assessment, diagnosis, treatment, and patient education, while working collaboratively with physicians in a team-based care model.
This is an excellent opportunity for an APP who values continuity of care, patient education, and working in a supportive outpatient clinic environment.

Responsibilities
Key Responsibilities
Provide comprehensive outpatient care, including patient assessments, diagnosis, and treatment planning
Perform physical exams, review diagnostic studies, and interpret laboratory results
Develop and implement individualized treatment and follow-up plans
Document patient care, assessments, and plans accurately and timely in the electronic medical record
Provide preventive care and health education to patients and families
Triage patient needs and manage care issues to improve patient outcomes
Collaborate closely with physicians and other healthcare disciplines to ensure quality and continuity of care
Serve as an advanced clinical resource within the clinic or specialty area
Demonstrate compassionate, patient-centered care across all age groups
