As a team member, you'll help guide clients through essential services that improve economic stability and well-being. Your work will make a meaningful impact across programs focused on health, housing, social services, and more. Be part of a team creating real change in the Bronx community. RESPONSIBILITIES: Provide direct assistance to individuals and families applying for or recertifying government benefits and entitlements. Conduct client intake, engagement, and assessment; develop service plans; provide referrals and advocacy; prepare exit summaries; and follow up to help clients secure benefits. Manage a caseload and connect clients to government benefits and entitlements that help them remain stably housed. Monitor and document client progress toward service plan goals in Salesforce Database. Document all interactions with, or on behalf of, clients in Salesforce database. Maintain familiarity with program resources available on- and off-site. Coordinate client services with other social services, mental health, medical, employment, educational, childcare, and other providers. Conduct regular case conferences with clients and service providers as required. Complete program reports monthly, or more frequently as required. Report to, and meet with, supervisor monthly. Conduct home visits, outreach events, and other fieldwork as required. Develop a customized service plan with clients, outlining action steps and responsibilities. Assist the client with obtaining necessary documentation for benefits and entitlements they want to acquire. This includes increasing the number of selected benefits, renewal of applications or the recertification process associated with the continuation of key benefits. Maintain up-to-date case-related applications and Salesforce database entries, as well as regular contact with clients through office visits, telephone contact, and home visits. Coordinate client referrals for other needed services Provide budget counseling to ensure effective money management skills. Perform other duties as assigned.
