The Doe Fund’s innovative social enterprise model creates additional opportunities for New Yorkers to gain paid, hands-on experience while delivering essential services across the city, including supplemental sanitation, graffiti removal, and rain garden maintenance. We are also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 2,100 apartments in operation or development, creating safe, dignified homes with supportive services and spaces that foster community and connection. The Doe Fund has an annual operating budget of $88 million and a full-time staff of over 500, a significant portion of whom are graduates of our programs.
We are seeking a Case Manager for The Doe Fund's permanent supportive housing for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 255-unit residence will provide housing for 153 individuals and families referred by the New York City HIV/AIDS Services Administration (HASA), and the remaining 102 units will be apartments for moderate-income households.
The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population.

Key Responsibilities:
Maintain a caseload and provide supportive case management services to these individuals
Maintain resident records in compliance with agency and funding standards
Conduct and review assessments
Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
Monitor and ensure residents are linked to healthcare providers
Provide counseling and crisis intervention as necessary
Assist residents in securing and maintaining benefits
Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
Facilitate groups and activities that promote health and independent living skills development
