The Doe Fund has an annual operating budget of $88 million and a full-time staff of over 500, a significant portion of whom are graduates of our programs. The Doe Fund’s Scatter Site Return Housing Program (SSR) focuses on assisting chronically homeless individuals in their transition from shelters to community living. The services include relapse prevention and crisis intervention using the principles of Harm Reduction and Motivational Interviewing, education on self-advocacy, community integration, safety, money management, socialization, employment skills, and referrals to appropriate treatment facilities. Through the Scatter Site supportive housing program, clients are moved into apartments located in the Bronx, Brooklyn, and Manhattan areas We are currently seeking a Clinical Case Manager. The person in this role will be responsible for providing supportive case management services including relapse prevention and recovery readiness to the Scatter Site Return clients. Key Responsibilities: Major responsibilities of the Clinical Case Manager include, but are not limited to the following: Participate in the intake process for all new clients by completing a mental health assessment. Develop and facilitate support groups within the program for clients. Provide supportive counseling and crisis intervention around a host of issues integral to living with mental illness/ substance abuse issues to clients and/or their families/significant others. Completes psycho-social evaluations for all clients. Serves as primary client contact to foster participation in the program. Documents client contacts and maintains a confidential record.
