The Complex Care and Recovery Program at the Centre for Addiction and Mental Health (CAMH) is currently seeking an full-time, temporary (6 months) Assessment Coordinator to work in the Adult Neurodevelopmental Services Outpatient. Reporting to the senior manager, the successful candidate will have a key role in engaging patients, families and service providers, their service needs and planning the interprofessional team’s response to their referral. This position supports a large-scale triage initiative, and combines direct contact with patients and families with hands-on management of the data that tracks them. You will monitor and participate in processing referrals made to the program and in booking clients’ appointments in collaboration with the medical head and the team, and you will be responsible for accurately recording and maintaining each client’s referral, contact and status information.
