Shift: First Shift (United States of America) Diabetes Educator - Certified or Non-Certified     Shift Details: Full Time Monday - Friday 8am - 4:30pm Position Overview: As a Diabetes Educator, you will play a fundamental role in helping hospitalized patients gain the knowledge, skills, and confidence needed to successfully manage their diabetes. Working collaboratively with providers, nurses, dietitians, and care teams, you will provide individualized education on blood glucose monitoring, medications, insulin administration, nutrition, lifestyle modifications, and diabetes technology. You will assess each patient's unique needs, develop personalized education plans, support discharge readiness, and help ensure a smooth transition to outpatient care. Encourage self-management that promotes the achievement of individualized behavioral and treatment goals that optimize health outcomes in persons with diabetes across the health care continuum. Diabetes Educator Required Qualifications: • Minimum of two year clinical experience required.  • Bachelor's degree in nursing (BSN) or Bachelor's degree in Nutrition/Dietetics, Exercise Physiology or closely related field required. • Excellent teaching and communication skills (written and verbal) required. • Ability to demonstrate initiative, teamwork and work with minimal direction required. • Public speaking ability required. • Basic knowledge in office computer tools such as Microsoft Office including PowerPoint, Word, and Outlook required.  •Basic Life Support (BLS) certification required, must be from an American Heart Association (AHA) endorsed program (effective for new hires starting January 1, 2023 or later). • Licensed…
