What You Will Do The Registered Dietitian provides individualized, age-appropriate medical nutrition therapy to pediatric and adult patients served by IHI, including individuals with bleeding disorders and nutrition-related comorbidities. Services are provided through clinic, outreach, telehealth, and approved community settings. As the organization’s nutrition subject-matter expert, the dietitian supports complex care, collaborates with the multidisciplinary team, and leads nutrition-specific program development and improvement in collaboration with clinical and operational leaders. The Opportunity Conduct comprehensive, age-appropriate nutrition assessments in clinic and outreach settings; document findings and incorporate recommendations into the patient’s individualized plan of care with the multidisciplinary team. Partner with patients and families to establish realistic nutrition goals, apply motivational and behavior-change strategies, provide individualized education and resources, and reassess and revise the nutrition plan through in-person, telehealth, or telephone follow-up. Conduct pediatric cardiovascular risk assessments in accordance with clinic protocols and within professional scope. Assessments may include height, weight, body mass index (BMI) blood pressure, lipid panel results, hemoglobin A1C, and blood glucose levels. Communicate finding and nutrition recommendations to the patient, family, primary care provider, and multidisciplinary team as appropriate, and coordinate follow-up based on identified needs. Independently manage nutrition care for pediatric and adult patients with rare, chronic, and medically complex conditions, using clinical judgment to prioritize needs and coordinate interventions. In collaboration with clinical and operational leaders, lead the design, implementation, and evaluation of the clinical nutrition program. Responsibilities include establishing nutrition goals and service-delivery workflows, identifying access strategies and resource needs, and evaluating program effectiveness with approved department priorities and available resources. Serve as the organization’s medical nutrition therapy subject-matter expert; consult with clinical disciplines and develop education for staff, providers, trainees, and community partners. Establish nutrition-related quality and performance measures, analyze outcomes and gaps in care, lead measurable improvement initiatives, and develop evidence-based protocols, clinical pathways, policies, and documentation standards with clinical and operational leaders.
