CV (Curriculum Vitae) must have all dates in MM/YY to MM/YY format Work History must be in Sequential order with most resent-on top Any Gaps over 30 days must be explained on the CV EPA (Envision Pre-Application) (Please use the attached EPA. It is also in Trio under your Name & then click on Help) All Licenses, Boards and Certs with expiration dates must be listed on CV Anyone with malpractice must provide a NPDB Anyone with legal issues must provide supporting documents Availability 1st date would they be available? How many shifts would they be able to do per month? What month is their schedule completely open? Any shift preferences (Day, Night, Blocksetc.)? Any dates in the next 3-6 months they cannot work? Any Holidays in the next 3-6 months would they be open to work.? The attached ICA must be used for any provider that is approved for credentialing.
