Please Note: This is a grant-funded position currently funded through June 30, 2027 .
This position includes travel to Los Angeles as needed
About the Role
The Project Manager Family Nurse Practitioner (PMFNP) provides both clinical and operational leadership for the Dr. Gloria J. McNeal Nurse Managed Clinic's community-based healthcare programs. The selected candidate will oversee clinic operations, supervise healthcare personnel, support grant initiatives, and deliver direct patient care to underserved populations.
This role serves individuals experiencing health disparities, including people experiencing homelessness and other at-risk populations , while also supporting healthcare services provided in community settings, including elementary schools .
Candidates must possess Family Nurse Practitioner (FNP) certification and have experience providing care across the lifespan.
Location & Travel
Candidates must reside in San Diego, CA
Frequent travel between clinic and community sites within the candidate's assigned region is required
Travel expenses are reimbursed
Ability and willingness to travel up to 75% to support clinic operations, outreach initiatives, and grant-related activities
What You'll Do
Clinical & Community Care
Deliver primary care services to underserved and vulnerable populations
Provide healthcare services to individuals experiencing homelessness and other at-risk populations
Support community-based healthcare initiatives throughout Los Angeles and San Diego
Deliver care within mobile, community, and school-based settings
Collaborate with interdisciplinary healthcare teams to improve patient outcomes
Monitor patient care plans and support continuity of care
Clinic Operations & Leadership
Manage day-to-day operations across multiple clinic locations
Oversee clinic workflows, scheduling, and patient-centered service delivery
Supervise Nurse Practitioners and Community Health Administration staff
Support billing, credentialing, compliance, and quality improvement initiatives
Manage Electronic Health Record systems and operational processes
Ensure culturally responsive and high-quality care delivery
