Position Title: Field Nurse (RN)
Department: Clinical Services
Reports To: Director of Nursing (DON)
Location: New Jersey (Field-Based)

Position Summary
The Field Nurse (RN) is responsible for conducting nursing assessments, developing care plans, supervising Home Health Aides (HHAs), and ensuring compliance with all New Jersey State regulations and agency policies. The Field Nurse serves as a liaison between clients, caregivers, physicians, and agency staff to ensure the delivery of quality home care services while promoting client health, safety, and independence.
Essential Duties and Responsibilities
Clinical Assessments & Care Planning
Conduct initial client assessments and reassessments in the home environment.
Develop, implement, and update individualized Plans of Care based on client needs and physician orders when applicable.
Evaluate clients' physical, emotional, and environmental conditions.
Identify changes in client status and communicate findings to appropriate parties.
Provide recommendations regarding services, safety concerns, and care needs.
Home Health Aide Supervision
Perform supervisory visits of Home Health Aides in accordance with New Jersey regulations and agency policy.
Evaluate aide performance and provide coaching, training, and corrective guidance as needed.
Ensure aides are delivering services consistent with the established Plan of Care.
Complete and maintain documentation of supervisory visits and competency evaluations.
Client & Family Education
Educate clients and family members regarding disease processes, medications, safety precautions, and available community resources.
Provide guidance on maintaining independence and improving quality of life.
Address client and family concerns professionally and promptly.
Care Coordination
Collaborate with physicians, hospitals, case managers, caregivers, and community agencies.
Communicate significant changes in client condition to the Director of Nursing and appropriate healthcare providers.
Participate in interdisciplinary care discussions when necessary.
Assist with transitions of care and service coordination.
Documentation & Compliance
Complete all assessments, supervision notes, care plans, and clinical documentation accurately and within required timeframes.
Maintain compliance with New Jersey Department of Health regulations and agency policies.
Ensure client records are complete, accurate, and audit ready.
Participate in quality assurance and performance improvement activities.
Qualifications
Required
Active and unrestricted Registered Nurse (RN) license in the State of New Jersey.
Graduate of an accredited school of nursing.
Minimum of one (1) year nursing experience.
Valid driver's license, reliable transportation, and current automobile insurance.
Ability to travel throughout assigned service areas.
Strong clinical assessment and documentation skills.
