Help us build the future of healthy aging Total Life is a national telehealth platform built for older adults. Today, we deliver behavioral health care to Medicare beneficiaries across 49 states through a network of 200+ credentialed providers. We were founded on a simple, personal belief: that the people who spent their lives caring for everyone else deserve care that actually meets them where they are. Now we're expanding beyond mental health into preventative, lifestyle, and longevity medicine — and we're hiring clinicians to help us build it. This is a ground-floor role. You won't be slotting into an existing clinic; you'll be shaping one. If you've ever wanted to design how lifestyle medicine should be delivered to older adults online — the protocols, the patient journey, the standard of care — this is that opportunity. Important: this is not a primary care role We want to be clear up front, because it shapes everything about how you'll practice here. You are not the patient's PCP. You won't manage acute illness, serve as the medical home, or replace anyone's existing physician. Instead, you'll work alongside patients and their care teams as a dedicated lifestyle and longevity partner — focused on prevention, behavior change, and the daily habits that determine how well someone ages. Think of the role as the missing layer in most people's care: the clinician who actually has time to help them navigate their health and wellness, set meaningful goals, and stick to them. What you'll do Partner with older adults to assess their health and wellness goals and build personalized, evidence-based lifestyle care plans Coach patients through sustainable behavior change grounded in lifestyle medicine — nutrition, movement, sleep, stress, connection, and reducing risky substances Anchor your practice in the 4 M's of Age-Friendly care — What Matters, Medication, Mentation, and Mobility — so every plan reflects what's most important to the patient in front of you Help shape the clinical model itself: protocols, intake, outcome measures, documentation, and the patient experience from first visit onward Collaborate closely with our behavioral health team to support the whole person, mind and body Coordinate with patients' existing primary care providers rather than duplicating their work The philosophy we practice by We're building this clinic on the foundations of lifestyle medicine — the discipline that treats daily habits as a primary tool for preventing, managing, and reversing chronic disease. That means grounding our care in the core pillars: whole-food, predominantly plant-based nutrition; regular, sustainable physical activity; restorative sleep; stress management; positive social connection; and avoidance of risky substances. We pair these with the 4 M's of Age-Friendly Health Systems — What Matters, Medication, Mentation, Mobility — so prevention is always anchored in each patient's own priorities and stage of life.
