As a Physical Therapist, you'll partner with primary care providers and an interdisciplinary team to address diverse musculoskeletal issues, support chronic disease management, and create lasting changes through movement and education. What You’ll Do (Key Responsibilities in Patient Care): Assessment & Planning: Conduct comprehensive musculoskeletal and screening assessments. Perform evaluations to establish functional goals, timeframes, and personalized treatment plans based on physician referral. Direct Intervention: Provide direct, high-quality care in-clinic or virtually, using adaptive equipment, therapeutic techniques, and modalities. Progress Management & Communication: Effectively communicate with patients, track progress, and regularly re-evaluate using standard tests. Revise goals and communicate changes to the physician as needed. Interdisciplinary Collaboration: Coordinate care within the interdisciplinary team, including goal/discharge planning. Provide necessary discharge instructions and home exercise programs to patients and families. Compliance & Outcomes: Adhere strictly to all regulatory, HIPAA, infection control standards, and company policies. Maintain strong performance on key company/patient care metrics (KPIs, NPS/PEX, and outcomes).
