Schedule: Part Time | 16 hours per week | Tuesday and Thursdays | 8:00am - 4:30pm Department: Participant Care | Holland, MI What You'll Do in This Role: The Pharmacy Technician provides daily pharmaceutical supportive services for the LifeCircles PACE clinic. The Pharmacy Technician will work closely with the Clinic Pharmacist, other members of the clinical team, the contracted PACE pharmacy, and other contracted partners (i.e., facilities) to assist in meeting pharmacy-related participant needs. The Pharmacy Technician may work independently in the community as well as on-site at the PACE facility. The Pharmacy Technician aids with medication setup, medication delivery, medication change notifications, and record keeping. Additional tasks will be assigned as needed by the Clinical Pharmacist or Clinic Department leadership.
Medication Distribution & Preparation
Assures efficient, timely, and accurate delivery and distribution of medications and medication orders
Assists in preparing alternate medication packaging as directed by the pharmacist (e.g., personal medication dispensers, medication wheels, plastic medication boxes)
Maintains accurate inventory of stock medication, ensures proper storage/security conditions, and maintains refrigerator temperature logs
Medication Compliance & Participant Support
Identifies and communicates issues with medication compliance to the pharmacist and/or clinical team in a timely fashion
Assists the Clinical Pharmacist with oversight of medication compliance
Performs home visits to assess and assist with medication compliance
Communicates regularly with participants, care partners, and the PACE contracted pharmacy to address medication refill needs and answer questions within technician scope
Transitions of Care & Coordination
Coordinates medication changes during transitions of care
Performs medication reconciliations during transitions of care
Documentation & Record Management
Ensures accurate charting and data entry into the electronic medical record

High school diploma or equivalent
Current Pharmacy Technician (CPhT) license with the State of Michigan
National pharmacy technician certification, preferred
Current CPR certification or willingness to obtain as offered by LifeCircles
Must have reliable transportation and a valid Michigan driver’s license
Minimum of one year of work experience, two years preferred
Basic pharmacology, including common medications, dosage forms, and strengths
Prescription processing steps and pharmacy workflow
Medication safety standards, labeling requirements, and storage guidelines
HIPAA regulations and patient privacy practices
State pharmacy laws and technician scope of practice
Strong attention to detail and accuracy
