About This Role: The Physical Therapist at Evara Health provides patient-centered care to help individuals improve mobility, restore function, and manage musculoskeletal and neuromuscular conditions. Working as part of a multidisciplinary team, this role develops individualized treatment plans and helps patients achieve their goals and improve their quality of life. **This posting is being used to build a talent pipeline for future opportunities at Evara Health. While there may not be an immediate opening, we encourage qualified candidates to apply so we can connect with you as positions become available.** What You'll do: Conduct comprehensive patient assessments and develop individualized treatment plans. Provide therapeutic interventions, including exercises, manual therapy, and patient education. Monitor patient progress and adjust treatment plans to support optimal outcomes. Educate patients and families on treatment plans, exercises, and strategies to support ongoing health and mobility. Collaborate with providers and other healthcare professionals to ensure coordinated, patient-centered care. Maintain accurate and timely patient documentation in accordance with professional and regulatory standards. Stay current with physical therapy best practices and incorporate evidence-based care. Collaborate with and mentor residents, students, and trainees while supporting clinical education and staff development. Provide high-quality, comprehensive care while promoting cost-effective practices. Maintain compliance with Evara Health policies and all applicable HIPAA, HRSA, state, federal, and accreditation requirements.
