Our client based in Worcestershire is seeking an experienced Social Worker to join their Front Door Team. This is an excellent opportunity for a dedicated practitioner to work with adults and carers at the first point of contact, promoting wellbeing, independence, and access to community resources through strengths-based and person-centred practice.
The successful candidate will support adults with complex and fluctuating needs, undertaking meaningful conversations, assessments, support planning, safeguarding interventions, and crisis response work. Working collaboratively with individuals, carers, health professionals, community resources, and partner agencies, you will play a key role in helping people regain control of their lives, reduce risks, and achieve positive outcomes within their local communities.
This role requires a confident and autonomous Social Worker with strong knowledge of Adult Social Care legislation, safeguarding, risk management, and multidisciplinary working.
What will your responsibilities be?
In this role, you will:
Undertaking strengths-based conversations, assessments, reviews, and support planning with adults and carers in accordance with the Care Act 2014.
Managing complex cases involving adults with fluctuating needs, increased risks, and crisis situations.
Supporting adults to maximise independence, wellbeing, and access to community resources and informal support networks.
Working collaboratively with partner agencies to ensure coordinated, multi-agency responses that promote positive outcomes.
Applying the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983 appropriately within day-to-day practice.
Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making processes.
Completing Section 42 safeguarding enquiries and applying the principles of Making Safeguarding Personal.
Assessing and managing risk effectively whilst promoting positive risk-taking and individual choice.
Identifying potential Deprivation of Liberty issues and ensuring the least restrictive options are explored prior to legal intervention.
Completing Continuing Healthcare (CHC) Checklists and contributing to Decision Support Tool processes where required.
Developing creative, outcome-focused Care and Support Plans that maximise independence and meet eligible needs.
Working with adults and carers experiencing crisis situations, helping them regain control and plan for the future.
Promoting the welfare of children and appropriately escalating safeguarding concerns where identified.
Representing the service and individuals at multidisciplinary meetings, reviews, safeguarding meetings, and legal proceedings where required.
Participating in duty functions and responding to referrals in line with service requirements.
Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.
