Care Corner Seniors Services' Active Ageing Group (AAG) is built around the Neighbour Network Movement (N2). We empower seniors to be fit and healthy, independent, and contributing. N2 shapes both how and why we work. It positions seniors not as recipients of care alone, but as active contributors who build and sustain their own neighbor networks. N2 is how the AAG aligns with and contributes to the National Age Well Neighbourhood priority, turning that national agenda into action at the neighborhood level through senior volunteerism. The Wellness Trainer delivers structured exercise and frailty interventions under the Frailty Management Programme (FMP) and Gym Tonic (GT) helping seniors build strength, balance, mobility, and functional independence. Working directly with senior participants, the role conducts assessments, facilitates individual and group sessions, monitors participant safety, and supports program outcomes through accurate documentation and engagement. In doing so, the Wellness Trainer helps deliver the fit and healthy, independent dimension of the AAG guiding principle, supporting seniors to stay active and contributing in the community. Key Accountabilities 1. Vision and Senior Empowerment (N2)
Deliver wellness programmes that help seniors become and stay fit and healthy, independent, and contributing, in line with the AAG guiding principle.
Support and guide senior volunteers assisting in wellness activities, in line with AAG volunteer development frameworks.
Encourage participants to progress from improved fitness towards active contribution in the community.
Model the AAG culture of senior empowerment in daily interactions with seniors, families, and volunteers.
2. GT Programme Delivery
Deliver structured GT exercise sessions following established protocols and safety guidelines.
Conduct individual and group sessions tailored to senior participants' functional abilities and health conditions.
Monitor senior participants' vital signs and physical responses before, during, and after sessions.
Guide senior participants in the safe and correct use of exercise equipment.
Support participants in improving strength, balance, flexibility, and cardiovascular health through progressive training.
3. FMP Service Delivery
Conduct participant screening and baseline assessments using established tools and health screening protocols.
Deliver structured interventions to improve functional mobility and muscle strength among seniors with mild to moderate frailty.
Support programme implementation in line with the FMP service model and operational guidelines.
Monitor participant progress and adjust exercise intensity based on capability and safety.
Collaborate with allied health professionals where needed to support participant rehabilitation.
4. Client Care and Safety
Ensure participant safety during all sessions through proper supervision and risk management.
