In this role, you’ll work directly with youth and families, bringing people and resources together to help them identify solutions and reach their goals. SALARY: $21.00-$23.00 per hour, non-exempt. Dependent on experience and qualifications. TRAVEL: Travel is required. Mileage reimbursement. Wraparound model by providing strengths-based, family-driven case management and coordination of services for children, youth, and their families. The C.A.R.E.S. Wraparound model is a child abuse prevention and diversion program designed to support families early in the child welfare continuum and help them build upon their strengths, resources, and natural supports. You will work directly with families to bring the appropriate people and supports together, help families identify their strengths and needs, develop a plan, connect them with resources, and stay involved to help the plan succeed. You will facilitate Family Team Conferencing and the Wraparound process, working collaboratively with families, youth, community partners, and referral sources to identify strengths and needs, develop individualized Plans of Care, and coordinate appropriate services and supports. You will promote a family-driven, youth-guided, community-based, culturally responsive, and trauma-informed approach.
